Houston Horned Frogs

Please be sure to print, fill out the waiver form and return to us by email or bring the form to all lessons.  The form must be completed in full for the child to be granted permission into the academy. Please remember that we are not responsible for any of the child's personal items (electronics, clothes, equipment etc.). If your child has belongs that he/she does not feel comfortable bringing, please leave those items at home or in their parents car. If the items are necessities due to health or other reasons, personally speak with anybody working the Academy and we'll store his/her items for them.
